KÖK GELİŞİMİ TAMAMLANMAMIŞ GENÇ DAİMİ DİŞİN MTA İLE KORONAL PULPOTOMİSİ: BİR OLGU SUNUMU
GİRİŞ
Kök gelişiminin henüz tamamlanmadığı, aşırı çürük olan genç sürekli dişlerde pulpanın vitalitesini korumaya yönelik çeşitli materyaller ve tedavi yöntemleri son yıllarda giderek yaygın olarak tercih edilmektedir. Bu olgu sunumunda amacımız, genç daimi dişin enfekte koronal pulpasının tamamen kaldırılması sonucunda kalan sağlıklı kök pulpasının kök gelişiminin ve vitalitesinin devam etmesini sağlamaktır.
OLGU SUNUMU
7 yaşında kız çocuk hasta kliniğimize diş ağrısı şikayeti ile başvurdu. Yapılan klinik ve radyografik değerlendirmeler sonucunda çürüğün pulpa dokusuna yakın olduğu, dişin kök gelişiminin devam ettiği, perküsyon hassasiyetinin olmadığı, sadece yemek yeme esnasında ağrı olduğu tespit edilmiştir. Çürük dokusu temizlendikten sonra perforasyona bağlı kanama durdurulamadığı için dişe MTA ile koronal pulpotomi yapılmasına karar verildi. 6 aylık takip sonunda kök gelişiminin devam ettiği gözlendi.
TARTIŞMA
Çocuk hastalarda vital pulpa tedavilerinin kök kanal tedavilerine kıyasla; daha az zaman, prosedürün göreceli basitliği, daha az postoperatif ağrı ve tedavi boyunca daha az sayıda radyografi çekilmesi gibi avantajları mevcuttur.
SONUÇ
Kök gelişimi tamamlanmamış genç daimi dişlerde uygun koşulları sağlayan vakalar seçildiğinde MTA gibi biyouyumlu materyallerle yapılan pulpotomi tedavisinin başarılı olduğu görülmüştür.
CORONAL PULPOTOMY OF YOUNG PERMANENT TOOTH WITH INCOMPLETE ROOT DEVELOPMENT: A CASE REPORT
ABSTRACT
In recent years, various materials and treatment methods aimed at maintaining the vitality of the pulp in young continuous teeth, where root development has not yet been completed, excessive caries and substance loss, have become increasingly preferred. In this case report, our goal is to ensure that the root development and vitality of the remaining healthy root pulp continues as a result of the complete removal of the infected coronal pulp of the young permanent tooth.
CASE REPORT
A 7 years old girl came to our clinic complaining of toothache. As a result of clinical examination and radiographic evaluations, it was determined that the caries was close to the pulp tissue, that the root development of the tooth continued, that there was no mobility and percussion sensitivity, and that there was only pain during eating. After removing the caries tissue, it was decided to perform a coronal pulpotomy with MTA on the tooth, as the bleeding due to perforation could not be stopped. At the end of 6 months of follow-up, it was observed that root development continued and the tooth was asymptomatic.
DİSCUSSİON
In pediatric patients, vital pulp treatments have advantages such as less time, relative simplicity of the procedure, less postoperative pain, and fewer radiographs during treatment compared to root canal treatments.
CONCLUSİON
Pulpotomy treatment with biocompatible materials such as MTA was successful when cases providing favorable conditions were selected in young permanent teeth whose root development was not completed.
