Abstract:

Does Radioactive Iodine Therapy Affect Thyroid Size and Tracheal Diameter? İntroduction and Purpose: Hyperthyroidism is a pathological condition in which tissues are exposed to excessively increased amounts of thyroid hormones. Three methods of treatment are used in hyperthyroidism: antithyroid agents, surgery and radioactive iodine (RAI) therapy. In general, surgical treatment is recommended for very large thyroids with tracheal compression. In this study, we aimed to determine whether RAI affects thyroid size and tracheal diameter. Materials and Methods: Thirty patients receiving 20 mci radioactive iodine for hyperthyroidism at Atatürk University Medical Faculty Department of Nuclear Medicine between March 2018 and February 2019 were included in this study. Neck MRI was performed before and 6 months after treatment. Thyroid and tracheal volumes of MRI were measured by Cavalieri method before and after treatment. The comparison of the values was made by dependent sample T test. Results: We found that 30 patients' thyroid volume decreased 36.06% after RAI. Thyroid volume reduction after RAI was found to be statistically significant (p0.0001). We found that the trachea volume increased by 12.76% after RAI. Tracheal volume increase after RAI was found to be statistically significant (p0.05). Conclusion: While RAI treatment reduces thyroid size, tracheal diameters increase after treatment. In our study, we used the Cavalieri method, which is an objective method and we found that the tracheal pressure disappeared and the trachea expanded. None of our patients experienced respiratory distress in the early period. Considering surgical complications and risks, large thyroids can be directed to RAI treatment instead of surgery.
