Giant pulmonary artery aneurysm (PAA) is a rare disease. We described a rare case of giant PAA due to uncorrected atrial septal defect with high pulmonary hypertension in a 35-years-old-woman. Her cardiac disease was first diagnosed at the childhood period, but she has not admitted to any hospital for follow up, till now. She denied dyspnea on exertion, cough and chest pain. Transthoracic echocardiograph revealed giant PAA and atrial septal defect with pulmonary hypertension. Multidetector computed tomography is used for confirmation of size, location and course of PAA.
