Sigmoid volvulus is the wrapping of the sigmoid colon around itself causing an intestinal obstruction. The initial treatment of the disease is endoscopic derotation in uncomplicated and non-gangrenous patients. To prevent a recurrence in successfully derotated cases, some practitioners apply an anal tube, which may cause some complications including bowel perforation. I comment about a paper, which presented a case of a successful endoscopic derotation of sigmoid volvulus with iatrogenic bowel perforation due to anal tube.
