Uterus didelphys with obstructed hemivagina and ipsilateral renal agenesis is a rare entity referred to as Herlyn-WernerWunderlich syndrome. It usually presents after menarche with remittent pelvic pain and a palpable pelvic mass due to hematocolpos. In this article, we present a 21-year-old patient with a term pregnancy presenting with uterus didelphys, obstructed hemivagina, hematocolpos, and ipsilateral renal agenesis. An emergency cesarean section was performed because of fetal distress. The fetus was in the left uterus, since due to hematocolpos, the right uterus was the size of 20 weeks of pregnancy. After delivery, the tip of ring forceps placed through the right cervix was not seen in the vagina. Right renal agenesis was diagnosed by abdominal CT taken after the cesarean section. Herlyn-Werner-Wunderlich syndrome is usually diagnosed after menarche, but in this case, it was diagnosed during a caesarean section.
