Organ transplantation is increasing all over the world and
immunosuppressive drugs are playing an important role during this
procedure. Several immunosuppressive drugs such as the calcineurin
inhibitors tacrolimus or cyclosporine and the mammalian target of rapamycin
inhibitors sirolimus or its derivative everolimus are used to prevent rejection
of the implanted organ in solid organ transplantation and suppress immune
reactions. The transplant recipients are exposed to traditional medicine and
herbal products for various reasons both during pre- and post-transplantation
period and they can alter the pharmacodynamics and pharmacokinetics of
immunosuppressive medication and leads to interactions. Drug interactions
can significantly alter the aim of the immunosuppressive therapy and may
lead to a reduction in the therapeutic effect of drugs, increased the risk of
serious adverse outcomes like organ rejection, or increase the effect of other
drugs that are used together. This can be harmful for the recipients and
patients should be informed regarding the potential hazards of such
interaction. The present review aims to summarize the mechanism of action,
indications of immunosuppressive drugs; also, to evaluate the potential interactions of these drugs with commercially available traditional and
alternative herbal medicines.
