Ebelerin Empatik Yaklaşımının Gebelerin Algılamış Olduğu Doğum Ağrısı ve Doğum Sonu Konforuna Etkisi: Kesitsel Araştırma

Özet

Amaç: Bu araştırma, ebelerin empatik yaklaşımının gebelerin algılamış olduğu doğum ağrısı ve doğum sonu konfora etkisini belirlemek amacıyla yapıl mıştır. Gereç ve Yöntemler: Tanımlayıcı tipte yapılmış olan bu araştırmanın ör neklemini, Mayıs 2023-Kasım 2024 tarihleri arasında Erzurum Şehir Hastanesi Doğumhane Kliniğinde çalışan 47 ebe ve ebe eşliğinde doğum yapan 352 primi par lohusadan oluşmuştur. Veriler, “Ebe Tanıtıcı Bilgi Formu”, “Ebelik Empati Ölçeği (EEÖ)”, “Lohusa Tanıtıcı Bilgi Formu”, “görsel analog skala [(visual ana logue scale (VAS)]” ve “Doğum Sonu Konfor Ölçeği (DSKÖ)” kullanılarak top lanmıştır. Veriler, araştırmacı tarafından doğumhanede görev yapan ebeler ve bu ebelerin doğumda yardım ettiği primipar gebeler olmak üzere 2 çalışma grubu üzerinde yürütülmüştür. Veri toplamak için ebelere tanıtım formu ve EEÖ dol durtulmuştur. Gebeler, doğumhaneye geldiğinde tanıtım formu uygulanmış, VAS ise doğum eyleminin latent ve aktif fazında doldurulmuştur. DSKÖ ve VAS (doğum sonu dönemdeki ağrıyı değerlendirmek), doğum sonu anne taburcu ol madan önce doldurulmuştur. Veriler SPSS for Windows 22 programı ile analiz edilmiştir. Bulgular: EEÖ ile DSKÖ’nün sosyokültürel konfor alt boyut ortala ması negatif yönde anlamlı bulunmuştur. EEÖ ve DSKÖ’nün fiziksel ve psikos pritüel konfor alt boyutları ile VAS latent, VAS aktif ve VAS postpartum dönem arasında anlamlı ilişki bulunmuştur. Ebelerin empati seviyesi yükseldikçe gebe lerin konfor seviyesi yükselmiştir. DSKÖ’nün sosyokültürel konfor alt boyutu ile VAS postpartum dönem arasında ve DSKÖ’nün toplam puanı ile VAS aktif ve VAS postpartum dönem arasında negatif ilişki olduğu belirtilmiştir. Doğum sonu konfor arttıkça VAS doğum eyleminin aktif fazında ve VAS postpartum dö nemde ağrı azalmıştır. Sonuç: Ebelerin EEÖ’den, gebelerin DSKÖ’den aldıkları puan ortalaması düşük bulunmuştur. Empatik yaklaşım sergileyen ebelerin, ge belerin konfor seviyesi üzerinde etkili olduğu bulunmuştur.
Objective: This research was conducted to determine the effect of midwives empathetic approach on the labor pain experienced by pregnant women and postpartum comfort. Material and Methods: This descriptive study was con ducted with a sample consisting of 47 midwives working in the delivery unit of Erzurum City Hospital and 352 primiparous postpartum women who gave birth with the assistance of these midwives between May 2023-November 2024. Data were collected using the “Midwife Descriptive Information Form”, the “Midwifery Empathy Scale (MES)”, the “Postpartum Woman Descriptive Information Form”, the “Visual Analog Scale (VAS)”, and the “Postpartum Comfort Scale (PCS)”. The study was carried out on 2 groups: midwives working in the delivery unit and primiparous women assisted by these midwives during childbirth. The midwives were asked to complete the Midwife Descriptive Information Form and the MES. For the pregnant women, the descriptive information form was administered upon admission to the delivery unit, and the VAS was completed during the latent and active phases of labor. The PCS and VAS (postpartum pain assessment) scales were completed before the mother was discharged after birth. The collected data were analyzed using SPSS for Windows version 22.0. Results: A significant neg ative correlation was found between the MES and the sociocultural comfort sub dimension of the PCS. Additionally, significant relationships were observed between the physical and psychospritual comfort subdimensions of the PCS and the VAS scores measured during the latent phase, active phase, and postpartum pe riod. As the empathy levels of midwives increased, the comfort levels of the moth ers also increased. A negative correlation was found between the sociocultural comfort subdimension of the PCS and the postpartum VAS scores, as well as be tween the total PCS score and both the active phase and postpartum VAS scores. As postpartum comfort increased, VAS pain decreased in the active phase of labor and VAS pain decreased in the postpartum period. Conclusion: The average scores of midwives on the MES and pregnant women on the PCS were found to be low. It was found that midwives who exhibited an empathetic approach had an effect on the comfort level of pregnant women.
