ENDODONTIC TREATMENT OF TWO-CHANNEL MANDIBULAR PREMOLAR TOOTH

Aim: Knowledge of the differences in root canal anatomies and canal morphologies is necessary for successful
endodontic treatment.1 Although lower premolar teeth usually have a single root and canal, root canal ataomy and
morphology can show wide variations.2-6 In some studies in the literature, this rate is 65.7-100%, the incidence of two
channels is 1.2- 11.7%, and the incidence of three channels is 0.4%.7-10
Case: To present the diagnosis and endodontic treatment of the lower first premolar tooth with two channels.
A 35-year-old female patient was admitted to our clinic with the complaint of spontaneous pain in the left mandibular
molar region. In the clinical examination of the patient, who was determined to have no systemic disease, a large coronal
caries lesion was detected in the first premolar tooth in the relevant region. On the radiographic examination, there is deep
dentin caries in the related tooth. Root canal treatment was started in the patient who was diagnosed with acute pulpitis
as a result of clinical and radiographic examination. After the canal preparation was completed, it was finished in a single
session using 30.04 gutta-percha and cold lateral condensation technique.
Results: It was observed that the tooth was asymptomatic in the radiographic images taken six months after the patient
and in the clinical examination. The follow-up process of the tooth is ongoing.
Conclusion: The etiology of endodontic failure is multifactorial, but one of the important causes is related to the root
canals that cannot be found. For this reason, along with the correct diagnosis and appropriate treatment planning, the
doctor should have a comprehensive knowledge of the variations in root canal morphology.
